Standard Health Services’ diverse staff serves people from varied backgrounds with a particular focus on providing access to affordable housing and resources.

Housing Consultation

Housing Consultation consists solely of developing the Housing-Focused Person-Centered Plan. Described in more detail on the DHS Housing Stabilization Services

Housing Transition

Supporting the person in applying for benefits to afford their housing, including helping the person determine which benefits they may be eligible for

Housing Sustaining

Developing, updating, and modifying the housing support and crisis/safety plan on a regular basis. Preventing and early identification of behaviors that may jeopardize continued housing

STANDARD HEALTH SERVICES HOUSING STABILIZATION SERVICES OFFERS SUPPORT TO THOSE WITH MEDICAL ASSISTANCE WHO ARE EXPERIENCING HOMELESSNESS OR HOUSING INSTABILITY.
